Go-Live Sign Off Form

Once the electronic medical record (EMR) solution is installed and configured to your satisfaction, you sign the Go-Live Sign Off Form which grants permission to turn on the EMR and begin using it.

The countdown on the 120-day warranty period begins on the day the solution is turned on and is being used in the clinic, not the day you sign the form.
