Apply
Physicians may apply for POSP funding if they join a clinic using a VCUR 2006 solution. For the new physician to be eligible the clinic must commit to transition to a VCUR 2008 solution. Please submit an application form, enrolment form, a signed Letter of Agreement and a Commitment to Transition form.

Change Notification
Physicians who wish to change their address, payment instructions or vendor information must notify POSP.

Leave of Absence
Physicians who wish to remain funded while they are on a parental, disability or sabbatical leave must submit a Parental, Disability or Sabbatical Leave Form.

Demographic Download
Physicians may request a download of their patient demographic data.

Phase 3 Reimbursement Form
Effective January 1, 2010, clinics using VCUR 2006 EMRs must complete a reimbursement form to receive POSP funding.
