Application Form

Clinics who are not enrolled in POSP must complete an application form to register the clinic and list the physicians who are applying to join POSP. A change management advisor will contact the clinic to help them with next steps.

Direct Deposit Information Form

The direct deposit information form must be completed by every physician who intends to implement a VCUR 2008 EMR solution. It lets POSP know where to deposit the physician's reimbursement and transition allowance funding.

Physicians who wish to remain funded while they are on a parental, disability or sabbatical leave must submit a Parental, Disability or Sabbatical Leave Form.

POSP Physician Participation Agreement

The POSP Physician Participation Agreement outlines the obligations physicians must meet if they wish to receive reimbursement funding and POSP support services.

Transition Allowance Application Form

The transition allowance is funding available to eligible physicians who are already using an EMR and who are transitioning to stay current with POSP by moving to a VCUR 2008 EMR solution offered by a qualified service provider.
